Brushing for Life Dental Packs Product Complaint Investigation

Update: 23/01/2019

  • We have now closed the return of goods; as stipulated in previous ICNs this date for return was the 14 December 2018. All products found still to be within trusts and the community should be disposed of immediately as a precautionary measure.

Update: 21/12/2018

  • Customers have been advised that a decision has been taken to withdraw all JA Marketing Brushing for Life packs. As outlined in the Product Recall ICN 541.
  • This withdrawal affects all Lot numbers for all Brushing for Life packs.
  • Customers were advised to:
    Cease use of all Brushing for Life packs with immediate effect.
    Contact their local NHS Supply Chain Customer Service Advisor to arrange return of all unused affected product in your possession including part boxes, using reason code 7 “Supplier Recall” for full credit.
    Return all requested products to NHS Supply Chain by Friday 14 December 2018.
  • Any products not returned by 14 December 2018 should be disposed of immediately.
  • If you require additional information, please contact your NHS Supply Chain Customer Service Advisor.

Previous Update: 27/09/2018

Change:

  • NHS Supply Chain has suspended MRA227 and MRA228 dental packs, following a customer complaint regarding the rubber foot on MRA227.
  • Whilst this is a separate complaint raised directly with NHS Supply Chain, the complaint is regarding the same product malfunction the supplier JA Marketing informed us of previously regarding MRA225 and MRA226. Please see the previous updates below for further information.
  • We are working with the supplier to assess batch control and identify all products with potentially hazardous feet.

Impact:

  • There are now four product codes currently suspended and unavailable to order:
    MRA225 - Mild Mint Brushing for Life, age 0-3 years, 1000ppm 50ml
    MRA226 - Mild Mint Brushing for Life, age 0-3 years, 1400ppm 100ml
    MRA227 - Mild Mint Brushing for Life, age 3-6 years, 1400ppm 100ml
    MRA228 - Mild Mint Brushing for Life, age 6 years and above, 1400ppm 100ml.
  • There are no direct or indirect alternative products available.
  • Please refer to the product listing below for further information on the suspended product codes.
  • Unfortunately we do not have an anticipated date for supply to resume.

Next Steps:

  • Customers are advised to read and follow product label instructions regarding child supervision.
  • We will update this ICN with further information on product availability as soon as we can.

Previous Update: 03/09/2018

  • JA Marketing has informed us that dental packs MRA225 and MRA226 have been independently reviewed and confirmed as meeting ISO standards.
  • Currently the dental packs are awaiting clearance from Trading Standards before supply can resume.
  • Unfortunately we do not have an anticipated date for supply to resume.
  • We will update this ICN with further information on product availability as soon as we can.

We have further information regarding the customer complaint that led to the suspension of product codes MRA225 and MRA226

A child was given an MRA225/MRA226 toothbrush; whilst chewing the bottom of the toothbrush, the child managed to remove the rubber foot. This resulted in a customer complaint.

JA Marketing's initial investigation has highlighted the following corrective actions;

  • MRA225 and MRA226 have been suspended from JA Marketing whilst they test the toothbrush design to confirm it meets industry ISO standards.
  • The supplier has confirmed that no other incidents have been reported and therefore a product recall is not required.
  • Trusts are advised to report any incidents via the standard NHS Supply Chain customer complaint route.
  • Customers are advised to read and follow product label instructions regarding child supervision.
  • We are actively working with supplier regarding the investigation and will share the outcome with our customers in due course.

Previous Update: 02/07/2018

Change:

  • The supplier JA Marketing has informed us of a product investigation for two Brushing for Life Dental Packs; MRA225 (age 0-3 years 1000ppm 50ml) and MRA226 (age 0-3 years 1400ppm 100ml).
  • The supplier is examining stock of MRA225 and MRA226 following a customer complaint.
  • NHS Supply Chain has decided to suspend these products whilst under review.

Impact:

  • Product codes MRA225 and MRA226 are currently suspended and unavailable to order.
  • There are no direct or indirect alternative products available.
  • However MRA227 (age 3-6 years 1400 ppm 100ml) and MRA228 (age 6+ years 1400ppm 100ml) remain available on the Brushing for Life Dental Packs framework.

Next Steps:

  • We are working closely with the supplier to resolve this issue.
  • We will share any further updates with customers regarding the supply disruption and anticipated timeline for supply to resume.
