- Completion of the requirements form does not bind your trust(s).
- The information you provide will be used to formulate the requirements to be presented through the DPS to the supplier.
- Your trust(s) will have the option to decline final participation in the call-off contract created under this DPS should the price or terms and conditions be unacceptable to you.

When you complete a requirements form a call-off exercise is undertaken, a pricing and contract template is sent to your NHS organisation. Once completed and returned to us, you will be able to purchase product(s).
