Organisations that provide community healthcare services, mostly for NHS patients but are not wholly owned or part of a Trust.

Organisations providing NHS and private dentistry services in England.

Registered charitable organisation.

Other arms length NHS organisations that support NHS services e.g. NHS Property Services / NHS litigation, BSA.

Organisations offering hard or soft facilities management services to NHS Trusts, that are not wholly owned subsidiaries of Trusts. Organisations offering managed services to NHS Trusts that are not wholly owned subsidiaries of Trusts.

Private Organisation, Care home, Pharmacy.

Private organisations that provide direct healthcare to patients e.g. private hospitals.

Organisations offering residential care for the elderly, or those suffering complex conditions.

Organisations that dispense or sell medicinal drugs and other healthcare products.

Privately owned organisations that do not provide direct healthcare e.g. wholesalers.

All sections must be completed, with as much information as possible.

As NHS Supply Chain delivers to a wide range of healthcare locations, we may need to undertake a road and access suitability assessment and confirm your preferred delivery point. If required, this assessment will normally be completed during the first delivery to your site.

Failure to provide sufficient and verifiable evidence of your eligibility to purchase Medicinal Products (MPs) will mean that you cannot place orders for these products, even if they remain visible within our catalogue.

Acceptable evidence includes one of the following:

• A copy of your MHRA Distributor’s or Manufacturer’s Licence (where applicable).
• A letter on headed paper containing, for example, a GMC, GDC or GPhC registration number, together with your forename(s) and surname.

As part of the requirements placed upon NHS Supply Chain by the MHRA as a distributor of Medicinal Products, we are required to carry out customer validation checks. We therefore reserve the right to restrict access to and supply of Medicinal Products to customers who have successfully completed the validation process.

Please note that supporting evidence will be subject to periodic revalidation. Failure to meet the validation requirements during a revalidation review may result in Medicinal Products being removed from your account access.

By submitting this application, you acknowledge and understand the following:

• Payment terms are 30 days from the invoice date.
• Orders below £120 will incur a £7.50 carriage charge (inclusive of VAT).
• Accounts are expected to be used on a regular basis, with a minimum ordering frequency of once per month. We reserve the right to review and close inactive accounts.
• Depending on order volumes, deliveries may be made using roll cages. The delivery location is responsible for ensuring that all roll cages are returned in accordance with NHS Supply Chain requirements.
• Products purchased through this account must only be used by or supplied to the organisation named within this application. Unauthorised use may result in the account being reviewed and access withdrawn.

Application Assessment

Upon receipt of your application, NHS Supply Chain will undertake a series of validation and approval checks which may include a soft credit search. This search is used solely for assessment purposes and will not affect your credit rating.

The submission of an application does not guarantee that an account will be opened. NHS Supply Chain reserves the right to approve or decline any application following its assessment processes and is under no obligation to provide an account, credit facility, products, or services to any applicant.
