Field Safety Notice Evolan Viscopaste PB7 (ICN 2391)

Alert:

• Evolan has issued a Field Safety Notice recalling a specific lot of their Viscopaste PB7.
• It has been identified that part of batch D300381 of the product Ichthopaste has been incorrectly packed in Viscopaste PB7 pre-printed folding boxes and distributed as Viscopaste PB7.
• Due to the incorrect labelling on the folding box, there is a risk that Ichthopaste is used on a patient instead of Viscopaste PB7.

Products Affected:

• The affected product is available from NHS Supply Chain:

• All stock within NHS Supply Chain has been checked and the affected lot quarantined.
• Future orders will be fulfilled with unaffected stock

Next Steps:

• Read and follow the full instructions in the Field Safety Notice.
• Cease use and quarantine any products with the affected Lot number D300381.
• Where affected product is identified, please also contact your NHS Supply Chain Customer Service Advisor to arrange a return for full credit (reason code 7 Supplier Recall).
• All affected products to be returned no later than Wednesday 24 January 2024.

Please be aware that in the event of a Field Safety Notice/Product Recall we may need to provide manufacturers, UK responsible Persons and Distributers with contact details of customers who have potentially received the affected stock. This is to help them to reconcile their stock and evidence to the regulators that all actions have been taken to ensure that the unused products have been removed from customers to prevent inadvertent use of faulty and potentially unsafe products.
