New Product Complaint Form – May Require ‘Whitelisting’

A small number of customers have contacted us to report difficulties accessing the new form. The advice from our IT team is to clear your website browser storage ‘cache memory’ and ‘cookies’ and try again.

Also, please ensure that your trust network administrator or IT department is not blocking the form’s URL (web page address) which is https://feedback.supplychain.nhs.uk/

We are introducing an improved customer Product Complaint Form for product and food complaints.

The new submission form has a cleaner interface and a faster loading time. It is easier to complete, with the introduction of new features and enhancements such as auto population. This will make submitting a complaint faster, simpler, and quicker to resolve.

The new form will be available from Monday 10 June 2024.

Please start using the new form – we will be updating the links to the Product Complaints Form on our website from 9am on Monday 10 June 2024.
